Original description
The description as published by the source.
pam_usb provides hardware authentication for Linux using ordinary removable media. In versions prior to 0.9.2, when updating a one-time pad file, a temporary file is created using open() without the O_EXCL flag. Without O_EXCL, the create operation is not atomic: two concurrent processes racing to update the same pad may both succeed in opening the file, with the second write silently overwriting the first. The one-time pad is the core replay-prevention mechanism of pam_usb. A successful race could result in the stored pad value diverging from what either process expected, potentially causing authentication failures or, in a precisely timed attack, creating a window for pad reuse. This issue has been fixed in version 0.9.2.

CVE-2026-48982 — CWE-362 / CVSS v3 5.8
Attack vector: local / no user interaction
Plan: 1) Audit SBOM/dependencies, 2) Stage→prod upgrade, 3) Add WAF/proxy monitoring on affected endpoints, 4) Hunt IOCs in logs.
Refs: see the GHSA / vendor advisory / patched release linked on this page.
A vulnerability (CWE-362) has been reported.
Check the vendor advisory for the affected versions.
Severity: Medium (CVSS 5.8/10). Data can be tampered with by attackers.
Update to the patched release per vendor advisory.
Until the patch is applied: disable the affected feature, apply WAF rules, or restrict access via network ACLs.
Search webserver/proxy logs for unusual request patterns matching this CVE's known IOCs. Audit SBOM/dependencies to find affected services.
